 Yes, our team is coming to the 2012 NSA May Mayhem. We will participate in the age division indicated below:

Enclosed is our $100 holding fee. This payment is due within 14 days and will hold a slot in the tournament until the tournament deadline, when full payment is due. All entries are "pending" and will not be confirmed until payment is received.

THIS IS A PAY AS YOU PLAY TOURNAMENT. EACH TEAM WILL PAY THE UMPIRES ON THE FIELD AT GAME TIME. YOUR ENTRY FEE DOES NOT INCLUDE UMPIRE FEES. UMPIRE FEES ARE $40 PER TEAM PER GAME. TEAMS MAKING IT TO THE CHAMPIONSHIP GAME DO NOT PAY THE UMPIRES FOR THAT GAME. THE TOURNAMENT HOST PAYS THE UMPIRE FEES FOR THE CHAMPIONSHIP GAME. YOU WILL NOT RECEIVE A CONFIRMATION OF ENTRY INTO THIS TOURNAMENT UNTIL YOUR DEPOSIT IS RECEIVED BY OUR OFFICE. ALL CANCELLATIONS ON OR BEFORE THE DEADLINE ARE SUBJECT TO A $50 CANCELLATION FEE--NO EXCEPTIONS. NO REFUNDS FOR CANCELLATIONS RECEIVED AFTER THE DEADLINE. YOUR SPOT CANNOT BE HELD UNTIL A DEPOSIT IS RECEIVED AND IS NOT GUARANTEED IF FULL PAYMENT IS NOT RECEIVED BY THE DEADLINE. PLEASE DO NOT RESERVE A SPOT IN THIS TOURNAMENT UNLESS YOU PLAN TO ATTEND.

NOTE: Teams are expected to be available to play games at any time on any day of the tournament as indicated by the tournament dates above. Tournament directors are under no obligation to honor special scheduling requests and may replace any team in any tournament that has required a special schedule as a condition of entry. Any team that forfeits two or more round robin games in one tournament will not be allowed to participate in the single elimination portion of the tournament.
