Your Camp Application and Student/Athlete Profile must be printed (before submitting) and mailed along with your signed permission slip and payment before your registration is complete. You will not receive email confirmation until payment is received.

NSA Worth Exposure Camp Permission Slip

I am the parent and/or legal guardian of ___________________________, and I agree to allow her to participate in all activities at the NSA Worth Exposure Camp to be held on Friday, August 13, 2010 in Dracut, Massachusetts. I understand that my child is not required to participate in this activity, but I grant permission for her to do so, despite the possible risks. I recognize that by participating in this activity, as with any physical activity, my child may risk personal injury. I hereby attest and verify that I am aware of the potential risks and that I have full knowledge of the risks involved in this event.

Showcase Sports and any chaperones or employees, servants, and agents of Showcase Sports, are hereby given authority to consent to any medical treatment that may be required by my above listed child in the place of and with the same authority as myself.

Showcase Sports and any sponsors, chaperones or employees, servants, and agents of Showcase Sports, are hereby released from any liability for all actions taken in good faith during the above listed NSA Worth Exposure Camp.

  • Money Orders ONLY
    • $80 for One Session/$150* for Two Sessions
    • Payable to Showcase Sports
    • NO REFUNDS -- Camp Held Rain or Shine
    • Paying by personal check will delay entry until check clears
*Student/Athletes registering for two sessions will receive one Camp t-shirt and one Camp number to use for both sessions.
